Please read the information below before completing the Facility Usage Form:

Thank you for your interest in collaborating with the Environmental Health Sciences Research Center for your proposed project. Please complete the Facility Usage form (link at bottom of page) to assist us in scheduling and prioritizing the use of our facilities and equipment and allocating staff time. The information you provide will also help us track and report the impact our Core Facilities have on environmental health sciences (EHS) research at the University of Iowa and beyond.
Before completing this form, please familiarize yourself with the services and/or equipment offered by the Core you plan to utilize.

This form also includes screening questions to determine if your project is a Clinical Trial (CT). Before completing this form, please review NIH guidance on what constitutes a CT. Please note that NIH interprets CT definition criteria more stringently than in the past – some studies that merely increase participants’ knowledge or change their thinking may be considered CTs.
If your needs change during the course of the project, the facility personnel may work with you to modify your facility usage plan (e.g. by making equipment available that was not initially requested).
If you have questions, please contact Rose Phillips, Center Coordinator, at rose-phillips@uiowa.edu.
